training collars + harnesses will be removed prior to entering our HQ to ensure safety while playing

there are inherent risks in Dog Ownership and in allowing your Dog to participate in activities at dogventures – starting with the simple fact that the predictability in dogs lies in their unpredictability. dogs, by nature, use their teeth, paws, and body to interact and communicate with one another. injuries can occur. furthermore, being outside comes with certain risks such as, but not limited to, trauma and injury, rattlesnake bites, ticks, fleas, wasp stings, and foxtails – and Owner accepts full financial responsibility for any/all related medical treatment to any of these. dogventures will use St. Helena Veterinary Hospital for emergencies. *

you must provide dogventures with proof of vaccinations for Rabies, Bordatella, and DHP before Dog will be allowed on site. failure to do so will lead to the cancellation of any reservations. moreover, there is no refund of money. *

i understand

your dog must be on monthly prevention for heart worm, fleas and ticks. *

for example: Sentinel (heartworm), NexGard (fleas, ticks)

i agree to disclose all medical conditions that have been diagnosed, including but not limited to, specific food allergies, respiratory and cardiac disease, orthopedic conditions, genetic disorders, and any medications the Dog(s) is currently taking. *

dogventures will not accept dogs that have had or have been exposed to a contagious, communicable disease and/or illness of any type during the past thirty (30) days before Dog’s scheduled reservation; OR have currently active diarrhea or who have had active diarrhea with the previous 14 days; OR possess a terminal illness. *

we accept cash, check, or VENMO. payment must be made prior to attendance. *

penalty fees. there is a penalty fee of $26.00 for every 15 minutes or part thereof that Client is late in picking-up Dog. this penalty fee begins at the time of closing at 1:31 pm. *

Cancellation Charge(s): the cancellation charge is the full cost of that day’s scheduled reservation. your reservation is non-refundable and non-transferrable. you miss it; you still pay for it. *

Cancellation by Client: any cancellation of a reservation by Dog Owner must occur by way of notice (phone call, email, or text) to dogventures during business hours, at least two (2) business days prior to the beginning of a schedule service of Dog. failure by Owner to provide the said notice shall result in the applicable “Cancellation Charge(s)” *

Cancellation by dogventures: any cancellation of a reservation by dogventures due to illness or personal emergency (excludes natural disasters) will be made known by email, phone or text. make-up dates will be offered for a 30-day period beginning upon regular service resuming. *

Natural Disasters: in the event of a natural disaster that effects our area dogventures will not be open. this includes, but is not limited to mandatory evacuations, evacuation warnings, poor air quality, or at the sole discretion of dogventures. our ability to notify Client is totally dependent on the safety of humans, animals, electrical power, and/or cellular capabilities. no refunds will be given; no make-up dates offered; no exceptions. *

in exchange for participation in dogventures, i agree to the following: *

i am the Owner or Caretaker of, and responsible party for the dog(s) named above (at top of form)

i agree, acknowledge and otherwise understand that there are certain inherent risks associated with dogs and with the above described activity and i assume any and all risk of: damages, injury or other harm caused by my Dog to other animals whilst in the care of dogventures, to all property of dogventures and to all agents, employees and other staff of dogventures; and disease, injury or other harm to my Dog that can occur while my Dog is under the care of dogventures staff including, but not limited to trauma and injury, rattlesnake bites, bee stings, foxtails, ticks, and fleas.

i understand that dogventures’ agents or employees are not licensed veterinarians and have no required background in animal medicine, illness or disease, and are not expected to diagnose, or otherwise discover, illness or disease that may afflict my dog while in the care of dogventures.

i acknowledge and otherwise understand that vaccines are not absolute protection for all contagious and harmful diseases that may afflict canines.

i agree to indemnify and defend dogventures against all claims, cause of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my dog.

i agree to pay for all damage to the property of dogventures and/or any of its partner locations caused by my dog.

i understand that dogventures is a separate business than and is not affiliated with the St. Helena Veterinary Hospital; and i agree to indemnify and defend St. Helena Veterinary Hospital against all claims, cause of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my dog.

any legal or equitable claim that may arise from participation in the above shall be resolved under California law or binding arbitration.
